Discharge Safely Service

For a successful discharge and to avoid readmission, it is important you get home safely, and you have everything you need to start healing. We will:

• Coordinate with facility staff and support your post-discharge care
• Provide a Caregiver to get you home safely
• Pick up your prescriptions, grocery shop and run errands
• Perform home safety evaluations and help you settle in at home
